使用显微外科血管夹系统暂时双侧闭塞四个主要子宫血管,用于腹腔镜下非常大的壁间子宫肌瘤的摘除术

本研究的目的是检查使用显微外科临时血管夹系统促进非常大的壁内子宫肌瘤腹腔镜摘除术的安全性、可行性和有效性。

在这项回顾性研究中,检查了 2017 年 9 月至 2020 年 3 月期间在两家三级转诊医院接受腹腔镜子宫肌瘤切除术和临时子宫血管夹闭治疗非常大(最大测量直径⟩≥⟩9 厘米)症状性壁内子宫肌瘤的 26 名患者的手术结果。 Titan 制造的血管夹 (YASARGIL® Aneurysm Clip System) 用于暂时闭塞双侧子宫动脉和子宫-卵巢血管。 主要结果包括手术时间、失血量、平滑肌瘤数量和体重、转化率、术中和术后并发症发生率以及住院时间。

包括 26 名患者。 主要的壁内子宫肌瘤直径为 9-22 厘米。 患者的一般特征相似。 平均手术时间和术中失血量分别为 175.3±32.7(范围 120-250)分钟和 241.1±103(范围 100-450)ml。 术后血红蛋白下降中位数为 0.89±0.75 g/dL。 没有患者需要输血。 没有手术转为剖腹手术。 没有发生重大的术中或术后并发症。

腹腔镜子宫肌瘤切除术可以安全有效地进行非常大的壁间子宫肌瘤,术中失血较少,使用血管夹暂时夹住双侧子宫血管。
