概要
腹腔镜胃切除术正在发展。随着肿瘤外科医生在胃癌微创手术方面的专业知识和经验不断增加,腹腔镜胃切除术的适应症正在扩大到晚期病例。许多研究已经证明了微创手术的好处,包括降低手术相关损伤的风险,减少失血,减少疼痛和早期康复。为了建立胃癌微创手术适用性的具体证据,许多多中心RCT正在进行,比较腹腔镜与开腹手术的短期和长期结果。腹腔镜胃切除术的进展正朝着越来越微创的方法发展,这些方法能够改善患者的生活质量,同时不影响肿瘤的安全性。

1.简介
尽管其发病率有所下降,但根据GLOBOCAN,2012年,胃癌仍然是世界上第五大常见恶性肿瘤,也是世界上癌症相关死亡的第三大原因(723,000例死亡,占总数的8.8%)。全球总发病率发生在东亚(主要是中国)[1]。特别是在韩国和日本,胃癌是最常见的恶性肿瘤之一,早期胃癌(EGC)的比例有所增加,部分原因在于全国监测[2,3]。鉴于早期胃癌的发病率增加,手术经验的持续积累以及随之而来的仪器化进展,腹腔镜方法已经变得更常用于治疗胃癌。自20世纪90年代以来,已经进行了腹腔镜手术,用于治疗淋巴结转移风险相对较低的患者的EGC。在这里,总结了历史转折点的有意义的手术(表1)。 Ohgami等人。 [4]报道了1991年使用病变提升法进行的腹腔镜楔形切除术,这是腹腔镜手术治疗胃癌的第一例。 Ohashi [5]在20世纪90年代早期也报道了胃肠道粘膜切除术。 1994年,Kitano等人介绍了成功的腹腔镜辅助远端胃切除术(LADG)和EGC淋巴结清扫术(LND)。 [6]。随着内镜治疗的发展,如内镜下黏膜切除术和内镜黏膜下剥离术,腹腔镜楔形切除术和胃内粘膜切除术的需求已经减少,而LADG与LND的应用对于治疗具有潜在风险的EGC患者有所增加。淋巴结转移。 EGC的腹腔镜手术基于几项前瞻性随机对照试验(RCT)而变得流行,这些试验通常报告改善的短期手术结果与开放手术的肿瘤学安全性相当[7-10]。

2.技术方面的进展
2.1.手助腹腔镜手术(HALS)
腹腔镜手术的技术难点包括器械的移动性有限和没有触感。然而,在HALS中,外科医生的左手通过一个约6-7厘米长的特殊加压套管插入腹腔,这样可以保持操作者的触觉[11]。结合腹腔镜辅助开放手术野外和高分辨率腹腔镜检查,HALS结合了腹腔镜手术和剖腹手术的优点。使用插入的手增加触感和深度感知可以解决LND胃切除术的几个技术难题[11]。在腹腔镜手术发展的早期阶段,它还具有发病率相对较低和手术安全性高的优点。 Kim等人。 [12]提出手助胃切除术可能是腹腔镜胃切除术初学者的一种很好的学习方法。但是,这种方法存在一些缺点。手可以侵入腹内工作空间,并且用于维持气腹的套管是昂贵的。此外,HALS在外科医生方面对人体工程学不利,导致肩部和前臂疲劳和外科医生的疲劳[13]。随着外科专业知识的不断发展,使用这种方法的外科医生数量明显减少。

2.2.腹腔镜辅助胃切除术
在腹腔镜辅助胃切除术(LAG)中,在腹腔镜完全动员胃后,通过上腹部的小切口在体外进行胃切除和吻合,以精确定位病变并确保重建。为了精确定位LAG中的病变,已经报道了内窥镜夹和腹腔镜超声检查的有效性[14]。在LADG延长LND期间使用三维(3-D)计算机断层扫描(CT)血管造影术进行术中导航的有用性[15],以及通过三维CT术前识别血管系统的重要性。血管造影[16]。

腹腔镜辅助全胃切除术(LATG)在重建和LND方面技术上要求很高,如11d,4sa和10 [3,17]。 因此,尚未概括使用LATG治疗上胃癌。 已经报道了使用圆形和线性吻合吻合的食管空肠吻合的各种重建方法。

2.3.完全腹腔镜胃切除术
在LAG中,通过小切口手术的手术过程有时难以进行,尤其是肥胖患者[18]。总之,腹腔镜远端胃切除术(TLDG)所有手术都是通过腹腔镜进行的,这使得追求微创性成为可能。当引入第一个三角吻合术时,TLDG受到关注[19]。已经报道了几种体内吻合技术,包括使用线性吻合器的Billroth II吻合术,β形Roux-en-Y重建,重叠法和使用线性吻合器进行全胃切除术和倒置T形吻合术后的半环[20-24] 。如今,完全腹腔镜手术是针对晚期癌症或残余病例进行的。 Shinohara等人报道了完全腹腔镜全胃切除术完成的第一例。 [25]。

2.4.重建方法
对于腹腔镜胃切除术的标准化,该程序不仅在LND方面而且在重建组件方面必须实现再现性,安全性和简单性。众所周知,Billroth I吻合术具有更简单的性能,更少的吻合口解剖变化,增加的生理途径,以及更低的粘连和内部突出发生率的优点。因此,它是韩国和日本远端胃切除术后最常进行的吻合术[26]。在腹腔镜手术期间,由于难以在十二指肠残端周围的狭窄工作空间中操作,因此体内胃十二指肠造口术通常被认为是具有挑战性的技术。因此,腹腔镜辅助重建手术已经普遍进行了很长时间。

2.4.1。三角吻合术
最近,人们普遍进行三角形吻合术,这是一种成熟的体内胃十二指肠吻合术。然而,它仍然需要操作员和助手的精密和精确的腹腔镜技术。该技术以下列方式进行。首先,使用内窥镜线性吻合器以与通常线成90°的角度横切十二指肠球。然后以常规方式分开胃。沿着胃和十二指肠的边缘形成小的进入孔。使用45mm线性吻合器近似胃和十二指肠的后壁。然后,仔细检查钉线是否有任何缺陷以确定吻合的颜色,之后用一个或两个线性吻合器闭合共同的进入孔[27]。关于吻合术最严重的问题是渗漏。吻合部位供血不足和张力过大可能导致吻合口漏。关于血液供应,delta技术尤其具有吻合缺血的潜在风险,因为必须解剖十二指肠残端周围的组织以为吻合准备足够的空间。在Billroth I重建中,残余胃的小尺寸可能导致吻合部位张力和渗漏[28]。因此,在小残胃或短十二指肠球的情况下,Billroth II或Roux-en-Y重建可能实现更安全的吻合。

2.4.2.Roux-en-Y吻合术
已知Roux-en-Y方法有利于术后生活质量(QOL),胆汁反流和吻合口漏的发生率较低[10,11]。然而,它更加费力,因为它需要创建两个吻合和一个十二指肠残端闭合。将空肠分成距Treitz韧带远端约15cm处的空肠和空肠空肠造口术,使用线性吻合器在空肠分裂远端30-40cm处进行。在空肠空肠造口术的产生之后,通过使用线性吻合器进行分开的空肠的远端部分和残余胃之间的侧向吻合。通过手工缝合的缝合线或使用线性缝合器[29]关闭公共入口孔。在Roux-Y胃空肠吻合术后几乎不会发生吻合口漏,因为吻合部位的血液供应良好且张力极小。同时,有报道称过长的Roux肢体会导致内部疝气或Roux淤滞综合征[29,30]。此外,尽管已知RY组中的反流罕见,但关于反流相关性胃炎的发病率是否不同仍存在争议。因此,没有证实任何特定方法的明显优越性。通过脐周小切口手术进行体外空肠空肠吻合术是普遍接受的。体内吻合术在非熟练和没有经验的外科医生的手中是耗时且技术上要求的,并且对于切除的胃的拔除,无论如何都需要至少3cm的切口。因此,一些外科医生更喜欢通过脐周小切口进行体外空肠空肠造口术以减少手术时间。

2.4.3。食管空肠吻合术采用圆形吻合器
由于难以实现安全的荷包缝合和在狭窄和深度手术区域中进行食管空肠造口术的技术挑战,与LADG相比,LATG或腹腔镜全胃切除术(LTG)的执行相对较少。

使用圆形吻合器的食管空肠造口术是开放手术中最著名的技术。对于LATG中的体外方法,进行4至6cm大小的上腹部切口以进入食管胃交界处。许多关于体外方法的报告用简单的术语描述了这些程序,例如“用钱包缝合线插入铁砧”。然而,这个程序通常非常困难,因为荷包装置太大而无法处理一个小的小切口空间,并通过狭窄的手术区域内的小切口缝合是一个严格的过程[31,32]。然而,已开发出具有较小头部的荷包弦,其可通过12mm端口插入腹腔中。因此,可以在腹腔镜视觉下进行荷包缝合[33](图1(a))。一些外科医生更喜欢体内手工缝制的荷包缝合线,因为它们相对简单(图1(b))。将安全的砧座插入食道残端是下一个具有挑战性的步骤。在荷包缝合后,用两个镊子打开食管残端,此时,可以轻松插入铁砧[34,35]。腹腔镜砧座插入的另一种方法是使用通过食道前壁的小食道切口附着有线的砧座。然后将线拉到外面以将砧座放置在食道边缘,并使用线性吻合器切割食道[36](图1(c))。引入了砧座(Orvil,OrVil TM,Covidien,Tokyo,Japan)的口腔插入作为降低手术复杂性的替代方案[37,38](图1(d))。尽管有顾虑,但会发生食道粘膜损伤,即经口砧通道的特殊并发症;这种粘膜损伤很少报道[39]。相机端口的位置和用于圆形吻合器插入的小切口尺寸对于在此过程中实现正确视图也是非常重要的。在体外吻合的情况下,大多数外科医生更喜欢上中线切口用于砧座插入。相同的切口用于从左下端口[31]或左上端口[40]插入带有腹腔镜的圆形吻合器。一些外科医生使用延长的脐切口[41]或左下口切口[37]。

2.4.4。食管空肠吻合术采用线性吻合器
LTG领域中线性吻合器的操作更容易,因为与圆形吻合器相比,线性吻合器更薄并且具有更好的顶端移动性。此外,无论食管直径如何,外科医生都可以进行线性吻合吻合术,并且与圆形吻合术相比,可以实现更大的吻合。此外,线性吻合吻合术有助于外科医生减少空肠的扭转,这是圆形吻合术的严重并发症。

对于重叠方法(图2(a)),在空肠肢体上的吻合器线远端约5cm处形成小入口孔,而在食管残端的左壁上制作另一个肠切开术。通过将吻合器插入食道肠切开术和空肠肢体的入口孔之间朝向腔的头侧进行吻合术。空肠肢位于食管残端的左侧。然后,使用体内手缝缝合线闭合共同的进入孔。对于功能性端端吻合术(FEEA)(图2(b)),食道在水平方向上进行体内横断。在空肠末端和横切食道的边缘处形成两个小入口孔。将线性吻合器插入孔中,并产生吻合。公共入口孔用线性吻合器封闭。

重叠方法和FEEA之间存在两个差异:食管空肠吻合的蠕动方向和关闭共同进入孔的方法。 FEEA中的食管空肠吻合术是在抗蠕动方向进行的;因此,空肠肢体需要在FEEA中比在重叠方法中进一步提升。另外,FEEA中的共同进入孔的闭合使用线性缝合器进行,而闭合使用重叠方法中的体内缝合进行。因此,由于在FEEA中需要比在重叠方法中更大的工作空间,因此必须在FEEA中执行围绕膈肌的解剖,这会增加膈疝的风险。相比之下,重叠方法需要先进的缝合技术,而体内缝合经常导致手术时间延长[42]。

2.5。腹腔镜功能保留手术
2.5.1.幽门保留胃切除术(PPG)
PPG最初用于治疗消化性溃疡,从那时起,它已被用作EGC的外科治疗,以保持功能并维持更好的生活质量[43,44]。根据日本胃癌治疗指南,PPG是胃中三分之一临床T1N0M0胃癌的治疗选择,距离幽门至少4.0厘米。需要仔细考虑从病灶到幽门的距离,因为短的胃窦可导致术后胃淤滞,这是PPG的典型并发症之一。考虑到对于EGC需要足够的远端切除边缘> 1cm,除了窦箍的长度之外,从病变到幽门的距离应该大于4.0cm。 PPG的标准技术包括保留迷走神经和肝内血管的肝分支,以允许幽门的结构和功能保存。在PPG期间通常省略站5的LND以保留迷走神经的肝分支。最后,使用手工缝合法或线性缝合器进行胃胃造口术。通过保留幽门功能,它可以带来潜在的营养优势,降低排便习惯的发生率,以及减少倾倒综合征和碱性反流等较少的胃切除术后疾病[45]。最近,据报道腹腔镜辅助PPG(LAPPG)在功能保持和微创性方面与传统PPG相比是有益的[46]。

2.5.2.迷走神经 - 保留胃切除术(VPG)
VPG旨在减少由迷走神经损伤引起的胃切除术后症状。它不仅保留了支配小肠的迷走神经后支的腹腔分支,还保留了支配肝脏和胆道的迷走神经前主干的肝分支[47]。然而,对于用于神经保护的不完整LND存在特殊关注。在LND期间,由于能量装置和牵引力可能发生非可见的神经损伤,使得难以在没有神经损伤的情况下完全去除淋巴结。即使神经被严重保留,也难以确认是否保留了神经的功能。此外,与传统胃切除术相比,需要更长的手术时间来解剖神经周围。尽管存在这些技术困难,但根据尚未发表的正在进行的研究,VPG继续使用神经监测等辅助方法进行。

2.5.3.近端胃切除术(PG)
在腹腔镜PG(LPG)中,重建类型可能是最重要的问题。食管胃造口术是最简单的重建方法,尽管大多数患者最终患有术后反流性食管炎和/或吻合口狭窄。为了减少吻合口狭窄和反流性食管炎的发生率,已经尝试了包括胃底折叠术,胃管形成术,幽门成形术和具有脚踝修复的食管固定术的抗流出手术。然而,这些努力的结果远远不能令人满意[48-50],并且已经引入了PG后食管胃造口术的两种有效替代方案,即空肠插入和双道重建(DTR)。引入空肠介入作为减少严重反流的策略;然而,由于空肠皮瓣和三个吻合术的固有技术复杂性,腹腔镜空肠介入并未经常进行,因此手术时间延长[51,52]。或者,DTR由三个吻合组成:Roux-en Y食管空肠造口术,远离食管空肠吻合术15cm处的胃空肠造口术,以及距离胃空肠造口术20cm远的空肠空肠造口术。安等人。据报道,用于近端EGC的DTR LPG显示出良好的术后效果,特别是在反流症状减轻方面。该程序还显示出改善营养状况,可接受的肿瘤学结果率,手术时间和并发症发生率的趋势[53]。该手术涉及通过吻合术创建额外的吻合术,胃空肠吻合术,其仅增加传统LTG吻合术程序5-10分钟。最后,延迟的胃排空变得不那么令人担忧,因为即使发生胃排空延迟,也存在食物的替代通道路径,与空肠插入相反。

2.6.单切口远端胃切除术(SIDG)
开发了减少端口或单端口腹腔镜手术以减少疤痕和手术压力,事实上,通过脐部的单孔腹腔镜手术不会留下任何可见的瘢痕。制作垂直2.5厘米大小的经脐切口,并将商业单口装置放置在脐切口中。不需要额外的辅助套管针。然后,使用灵活的范围来确保操作区域的清晰视图。为了有效解剖,在较小曲率侧和胰腺上区域操作时使用弯曲器械。关于胰腺外LND,胰腺的颈部和身体有时过度突出,这使得难以使用来自脐部的直的器械解剖胰腺后面的淋巴结。由于不断需要更先进的技术和肿瘤学安全的疤痕证据,SIDG尚未推广。此外,尽管先前的SIDG报告显示可比较的发病率和缺乏开放转换,但纯粹的单端口手术中意外的术中事故的风险不容忽视。需要进行大规模研究以确认SIDG的安全性和肿瘤学结果。

2.7.机器人胃切除术
手术机器人系统于2000年初实施,以克服腹腔镜手术的缺点。机器人系统的特征,如三维视觉,消除生理性震颤和关节臂,通过促进手术领域的操作来协助外科医生[54]。铰接式内窥镜腕部允许操作员通过180°铰接和540°旋转实现七个自由度。此外,由操作员控制的放大的3-D高清成像系统和非常稳定的相机平台是重要的特征。机器人手术的另一个优点是能够实现更精确的体内缝合,从而极大地促进了全胃切除术后的重建[55]。通过使用萎缩器械,可以更容易地进行心脏LND的精细解剖,远端食管的动员以及将砧头插入食道残端。此外,由于其简单性和早期适应的潜力,机器人手术的学习曲线非常陡峭[56,57]。最近,已经报道了具有图像引导辅助的机器人手术。 Kim等人。 [58]证明操作者可以通过术中图像引导功能使用血管图并避免血管损伤或对其他器官的损伤。在机器人胃切除术期间,精细运动和放大视图允许外科医生在没有任何血管损伤的情况下执行清晰的LND并且具有最小的术中出血并且保留脾血管的小分支。

3.临床结果
3.1.发病率和死亡率
据报道,在LDG中,手术并发症的发生率总体上低于常规ODG。在之前的前瞻性RCT中,作者报道LADG与开腹胃切除术相比具有明显的肺部并发症更少的优势[59]。此外,对5项随机对照试验和17项非随机对照试验和3411名患者的荟萃分析报告,LADG可能导致失血减少,镇痛药消耗减少,住院时间缩短,而且总住院费用没有增加[60]。而Kim等人。 (COACT 0301)[61]报道LADG轻度并发症显著减少(LADG与ODG; 23.2%对41.5%,p = 0.012),韩国腹腔镜胃肠外科研究(KLASS)组进行了3期多中心RCT(KLASS- 01)建立更强有力的证据,最近报道了他们的发病率和死亡率数据[62]。 LADG组的总并发症发生率显著降低(LADG与ODG; 13.0%对19.9%,p = 0.001);特别是,LADG组的伤口并发症发生率显著低于ODG组(3.1%对7.7%,p <0.001)。然而,两组之间的主要腹腔内并发症(7.6%对10.3%,p = 0.095)或死亡率(0.6%对0.3%,p = 0.687)无显著差异。对于晚期癌症病例,一个中国组最近报道了与常规ODG相比,LDG的手术安全性为D2 LND [63]。在他们的多中心前瞻性RCT中,未显示术后发病率的显著差异(LDG与ODG; 15.2%对12.9%,p = 0.285)。两组的死亡率也相似(LDG与ODG; 0.4%对0%,p = 0.249)。

相比之下,只有少数研究报道了LTG的可行性和安全性,结果仍存在争议[40,64-66]。一些作者报告说,LATG提供技术可行性和安全性,与开放性全胃切除术相当。 LATG也显示与较少的术后并发症,较少的疼痛和快速恢复相关。然而,结果的异质性可能是由以下限制引起的:研究的主要回顾性,小样本量,大多数EGC病例和短期随访期。

最近,一些回顾性研究报道TLDG技术上可行,创伤小,比LADG更安全[67-70]。在一项包括652例患者的5项研究的荟萃分析中,TLDG与LADG相比,术中出血量减少,术前早期肠胃张力降低,术后并发症发生率降低。作者小组进行了一项前瞻性随机对照试验,以评估TLDG的总体可行性[71],其中显示TLDG与LADG一样安全可行,并发症发生率相当。实际上,如果没有作者的期望,TLDG和LADG组在术后病程如恢复,术后肺功能和炎症参数方面没有显著差异。这可能是临床领域中用于评估早期手术结果的参数无法准确反映TLDG和LADG之间手术侵袭性的细微差异的原因。

最近,韩国机器人胃切除术研究小组报告了一项多中心前瞻性临床试验的结果,该试验比较了EGC中机器人胃切除术与腹腔镜胃切除术[72]。患者入选机器人(n = 185)或腹腔镜(n = 185)胃切除术。该研究显示类似的整体并发症发生率(11.9%对10.3%;机器人与腹腔镜)和主要并发症发生率(1.1%对1.1%;机器人与腹腔镜),两组均无手术死亡率。在机器人手术组中,与腹腔镜组相比,手术时间明显更长(221分钟对178分钟;机器人与腹腔镜,p <0.001)。两组之间在开放转换率,手术失血量,停留时间或饮食积累方面没有显著差异。作者得出结论,机器人胃切除术并不优于腹腔镜胃切除术。确实,机器人组的成本较高[韩国赢得了13,748,422.5(13470美元)(机器人),而9,165,862(8980美元)(腹腔镜); P <0.001],但由于折旧和维护成本,大部分高成本在未来不会成为大问题。作者应该研究需要机器人胃切除术的适应症,在必要时应用机器人手术,并通过减少并发症来减轻社会负担,而不是简单地计算每次手术的费用。

3.2.生存和复发
关于肿瘤学安全性,作者小组报告了LADG对EGC的长期安全性[73]。在55个月的中位随访期间,肿瘤复发率为0.9%,癌症相关死亡率为0.5%(仅1例患者),LADG和ODG的总体5年生存率无显著差异组(95.9%对94.9%; LADG对ODG)。曾等人。 [60]也报道了两组间荟萃分析的长期存活率相当。前瞻性RCT(COACT 0301)的长期结果显示,LADG和ODG组的存活率具有可比性(5年总生存率:97.6%对96.3),其中164例患者的中位随访时间为74.3个月。 ,p = 0.721; 5年无病生存率:LADG与ODG的98.8%对97.6%,p = 0.514)[61]。

已有多项关于腹腔镜胃切除术和晚期胃癌(AGC)延长淋巴结切除术的肿瘤学结果的研究。这些研究的结果表明,腹腔镜手术的3年总生存率(75.3%)和无病生存率(69.0%)与传统开放手术的已发表研究相当[74-76]。

两项多中心随机对照试验,即KLASS-01研究和日本临床肿瘤学组(JCOG)0912试验,目前正在进行大规模研究,并试图阐明腹腔镜胃切除术对EGC的肿瘤学结果。 KLASS-01试验是第一个在临床I期胃癌患者中比较腹腔镜手术和开放手术的多中心RCT。总生存率是主要终点,次要终点包括发病率和死亡率,无病生存率,生活质量,炎症反应和成本效益。 2006年至2010年共招募了1416名患者(分别为LADG和ODG组中的705名患者和711名患者),最终结果将在不久的将来报告[77]。 JCOG于2010年启动了一项多中心RCT,将来自33个机构的920名I期胃癌患者的LADG与ODG进行了比较。在JCOG 0912研究中,总生存率是主要终点,次要终点包括短期临床结果,不良事件,LADG完成比例和开放手术转换,无复发生存和术后生活质量[78]

3.3.生活质量
腹腔镜胃切除术与开腹胃切除术相比具有许多优点,例如疼痛减轻,术后住院时间缩短,恢复时间更长,美容效果更佳。在几项荟萃分析中,显示了腹腔镜胃切除术在术后恢复方面的优势[86,87]。在另一份报告中,LADG显示出改善的短期症状和功能结果[61]。

引入功能保留手术以改善患者的QOL。最近,将LAPPG的营养和功能益处与LADG的营养和功能益处进行了比较[46]。据报道,尽管LADG中其他并发症的发生频率高于LAPPG,但胃排空延迟的发生率较低。与LAPPG相比,LADG术后1至6个月血清白蛋白和蛋白水平降低,术后1年腹部脂肪量增加。作者得出结论,LAPPG在胆结石发病率和营养优势方面较低,可被认为是中第三EGC优于LADG的优良治疗选择。此外,一项随机对照试验报告,接受VPG治疗的患者在12个月时腹泻明显减少,食欲减少。他们得出结论,与常规胃切除术相比,VPG可以改善术后QOL [47]。

在近端EGC中,PG的应用一直受到限制。在一项比较TG与PG的系统性荟萃分析中,PG与食管胃造口术相比,反流性食管炎和吻合口狭窄的发生率更高[88]。然而,已经报道了具有改良重建方法的LPG的几个阳性结果[48,89]。因此,LPG可被视为近端EGC的一种有吸引力的治疗选择,作为保护功能的微创手术,包括减少术后不适,预防贫血,改善营养和改善肠道激素的产生[90-93]。

4.结论
