How tight is tight? Many surgeons do sleeves at 32 Fr, claiming better weight loss. However, leaks are higher whenever sleeve size if below 40 Fr, without much change in weight loss. The ideal staple line should be straight, and accommodative of a wide bougie, going through the tips of the vessels coming in from the lesser curve. The last firing should spare the GE junction (at the top).
